Applying for Medical Report?

The Medical Records Department (MRD) at Mawar Medical Centre facilitates requests for:

  • Written medical reports
  • Investigative reports
  • Completion of insurance, SOCSO, and EPF forms
1.0 How to Apply for a Medical Report

To request a medical report, follow these steps:

  1. Download and complete the Medical Document Request Form.
  2. Obtain written consent from the patient for the release of medical information. If the patient is under 18, mentally incapacitated, or deceased, consent must be provided by a parent, guardian, next-of-kin, or legal representative.
  3. If applicable, download and complete:
    a. The Authorisation to Collect Patient Medical Record Form or
    b. The Authorisation to Release Patient Medical Report Form
    d) Refer to Section 2 – Required Documents for a list of necessary supporting documents.

To process your request, please provide the following documents. Submission of documents can be by email or by hand to the Medical Records Department. Refer Section 3 for information on the email and address.

  • Completed Authorization to Release Patient Medical Report Form (must include the patient’s signature as proof of consent and authorization).
  • Photocopy of the patient’s ID (NRIC or Passport).
  • Photocopy of the requestor’s ID (NRIC or Passport), if the requestor is not the patient.
  • Where applicable: A formal application letter, such as a Query Letter, Insurance Claim Form, Lawyer’s Letter, SOCSO form, etc.

You may submit your request through any of the following methods:

  • In Person: Visit the Medical Records Department Counter (Medical Records Office, Level 1).
  • By Email: Send your request to medicalrecord@mawarmedical.com.
  • By Mail: Address your request to:

Standard Medical Reports and Insurance Claims are typically ready for collection within 2 weeks after all required documents and payment have been received. However, some requests may take longer due to the following factors:

  • Patient is currently admitted/hospitalized
  • Doctor’s availability (e.g., on leave)
  • Visiting doctors without clinic sessions at Mawar Medical Centre
  • Upcoming clinic appointments for the patient
  • Multiple reports requested from different doctors
  • Doctor’s limited consultation sessions at Mawar Medical Centre (e.g., only two sessions per week)
